2 Inpatient/Outpatient Pharmacists - Seattle, WA

Description:Hours to be worked: Most incumbents will be assigned to the day shift, Monday – Friday, 8am to 4:30pm for full-time and selected days, 8am to 4:30pm for part time. In some cases, VA may need an incumbent to work other shifts and on weekends. This can be negotiated on a case by case basis. Experience: Prefer VA trained pharmacist with Vet Pro certification. One year experience as a registered pharmacist is required. Competencies: VA will need a copy of the incumbent’s competency assessment completed by the contractor. General Description for Duties: The pharmacist provides pharmaceutical care to VA Puget Sound Health Care System patients. Pharmaceutical care is the mission of the pharmacy profession and the Pharmacy. It is rational, effective drug therapy that improves a patient's quality of life. Pharmaceutical care is patient-focused and encompasses all aspects and processes involving medications. It integrates the knowledge and skills of pharmacists and technicians working in a system of drug use control, and promotes acceptance of individual responsibility for patient outcomes. The incumbent promotes delivery of pharmaceutical care service programs for all patients seen in medical center clinics, on inpatient wards, and central inpatient or outpatient pharmacies. The pharmacist provides outpatient dispensing, prescription verification, checking, unit dose drug distribution, controlled substance and high-cost drug accountability, drug information, formulary management, consulting, antimicrobial consulting. The incumbent recommends optimal drug selection and therapy to improve a patient's quality of life. The pharmacist provides patient medication counseling by verifying patient understanding of the medication, its intended purpose, proper use, expected outcomes, how to obtain more medication and how the current prescription will be processed (mail or pick up); identifies actual and potential drug related problems and works with other professional staff to resolve and prevent actual drug related problems; participates in the provision of pharmaceutical care support services as required including drug distribution, computer prescription processing, checking and documentation of interventions. The incumbent must foster a service culture of customer-driven, patient focused care; anticipate customer medication needs; initiates and maintains effective and timely service regarding medication dispensing. The pharmacist must works with other members of the interdisciplinary patient care teams to ensure all customer needs are met, internal and external and provides drug information support to other health care providers regarding formulary management issues and appropriate drug therapy. Additional Info:The contractor Shall be VetPro certified prior to employment at the Portland and Vancouver VA medical centers.
