8+ Weeks - Pharmacist - Fountain Valley, CA

Location: Fountain Valley, California
Term of Assignment: 8+ weeks; temporary assignment
Shift: 20 hours weekly
Pharmacy Hours: Monday to Friday, 9:00 am - 5:30 pm
License Requirement: Must be licensed in both Oregon (OR) and California(CA)

Responsibilities:

1. Review and monitor IV medication therapy for patients in home care and other outpatient settings.
2. Ensure effective communication with providers, patients, and associates to provide patient-focused accuracy, safety, and security.
3. Provide patient consultations regarding the safe use and storage of prescription medications, relevant health factors, and respond to inquiries.
4. Consult with prescribing physicians as questions warrant.

Requirements:

1. Must have a valid pharmacist license in good standing in both California (CA) and Oregon (OR).
2. Specialty or infusion pharmacy experience is preferred.
