Key Responsibilities

• Conduct phone-based medication reconciliation to ensure patients are on the correct prescriptions.
• Process medication refills and address patient concerns regarding their prescriptions.
• Collaborate with physicians to resolve medication-related issues and formulary selections.
• Review lab results to monitor therapy effectiveness and prevent adverse reactions.
• Educate patients and healthcare staff on medication use and best practices.
• Ensure compliance with inventory records and controlled substance regulations.

Qualifications

• Active Ohio pharmacist license in good standing.
• Inpatient hospital pharmacy experience required.
• Experience in cardiovascular medicine is required.
