Responsibilities:

• Conduct comprehensive medication reviews and assessments.
• Collaborate with healthcare teams to develop and implement patient care plans.
• Provide drug information and education to patients and healthcare providers.
• Ensure compliance with all relevant laws, regulations, and standards.
• Participate in quality improvement initiatives.
• Compounding production.
• Team supervision.
• Order fulfillment.
• Counseling patients.
• Verifying RXs
• Billing

Qualifications:

• B.S. in Pharmacy or Pharm D.,
• NY state Pharmacist license.
• At least 3-5+ years as a compounding pharmacist
• MTM experience is preferred.
• New Grads are welcome.
