Day Shift Staff Pharmacist - Wilkes-Barre, Pennsylvania

RPh on the Go is seeking a dedicated and detail-oriented Staff Pharmacist to join our team at a Long Term Care facility in Wilkes-Barre, PA. This full-time, day shift position offers a unique opportunity to make a meaningful impact on the lives of residents by ensuring they receive the highest standard of pharmaceutical care.

Key Responsibilities:

• Dispense medications accurately and in a timely manner.
• Review and interpret physician orders, ensuring they are appropriate for the residents.
• Monitor drug therapy for drug interactions, contraindications, and therapeutic duplications.
• Collaborate with healthcare professionals to develop and modify patient care plans.
• Provide patient and caregiver education on medication use, safety, and potential side effects.
• Maintain accurate records of all medications dispensed and pharmaceutical activities.
• Adhere to all regulatory requirements and facility policies and procedures.
• Participate in ongoing professional development and training to stay current with industry standards.

Shift:

• Monday - Friday: 8:00am - 5:30pm
• Rotating weekends.

Qualifications:

• Pharmacy experience
• LTC experience a plus
• PA pharmacy license
