Full-Time Retail/Long Term Care Pharmacist - Syracuse, NY

We are seeking a licensed Pharmacist for a direct-hire opportunity in a retail/long-term care pharmacy setting in Syracuse, NY. The ideal candidate will have prior long-term care experience, but experienced retail pharmacists from high-volume environments will also be considered.

Schedule:
Must be flexible to work both day and evening shifts, with start times as early as 7:00 AM and end times as late as 11:00 PM.

Rotating weekend schedule:

• Saturday: 8:00 AM 5:00 PM
• Sunday: 7:30 AM 4:00 PM

Responsibilities:

• Enter and review faxed, verbal, electronic, and paper orders.
• Perform Initial Review (PV1) and Final Review (PV2) of orders.
• Communicate with physicians for prescription clarifications.
• Manage EKits and Automated Dispensing Machines (ADS).
• Process new and completed backorders.
• Handle controlled substances, including logging and pulling.
• Enter and compound IV/TPN and non-sterile compounds.
• Make necessary NDC changes and check pre-packed medications.
• Process OOP/LOA requests and triage documents.
• Complete Quality Assurance (QA) sheets as needed.
• Process orders for REMS medications (e.g., clozapine).
• Conduct clinical reviews of patient profiles and make clinical decisions.
• Perform on-call duties as scheduled.
• Report medication errors and communicate with the billing department as needed.
• Receive and enter 222 forms from facilities.
• Perform pharmacy opening and closing duties.

Requirements:

• Experience: 1-2 years (flexible, must be open to pay range).
• Software: Proficiency with Frameworks and DocuTrack.
• Licensure: Active and good-standing New York State Pharmacist license.
