Full Time Staff Pharmacist - First or Second Shift Availability

Key Responsibilities:
Review medication orders and patient profiles to ensure accuracy, appropriateness, and compliance with regulations and standards of practice.
Monitor medication therapy for potential drug interactions, adverse effects, and therapeutic duplications.
Collaborate with healthcare team members to develop and implement individualized care plans for residents.
Provide medication counseling and education to residents, families, and caregivers.
Oversee medication dispensing, distribution, and inventory management processes.
Participate in medication reconciliation and medication therapy management programs.
Maintain accurate and complete documentation of pharmaceutical care activities.

Qualifications:

• Active Pharmacist License in the state of Kentucky
• DocuTrack/Frameworks Experience a plus
• Previous LTC Pharmacy Experience a plus
