We are looking for an experienced Clinical Pharmacist to join our team for a rewarding 13-week contract in Homer, Alaska. This role offers a unique opportunity to advance your clinical pharmacy skills while working in a remote, tight-knit community surrounded by the breathtaking beauty of Homer.
Responsibilities:
- Medication Order Review: Ensure the accuracy and appropriateness of medication orders across various hospital departments.
- Sterile Compounding & Verification: Prepare and verify sterile medications in accordance with strict pharmacy standards.
- Pharmacy Inspections: Conduct pharmacy inspections in clinic settings.
- Clinical Pharmacy Services:
- Acute Care Pharmacy: Medication verification, drug therapy management, lab reviews, sterile product preparation, and discharge counseling.
- Primary Care Pharmacy: Offer drug information, perform chart reviews, authorize refills, manage anticoagulation therapy, and administer vaccinations.
- Long-Term Care (Quyanna Care Center): Review medication regimens, provide consultations, and prepare prescriptions.
- Outpatient Pharmacy: Verify prescriptions, dispense medications, counsel patients, and offer vaccination services.
- Village Health Services: Support village clinics with prescription entry, medication management, and conducting clinic inspections.
- Collaboration: Work closely with physicians to optimize drug therapy and improve patient outcomes.
- Formulary Management: Assist with formulary reviews and monitor drug utilization for cost-effectiveness.
- Education & Mentorship: Mentor pharmacy students, educate staff, and participate in Pharmacy and Therapeutics (P&T) committees.
Qualifications:
- Active Pharmacist License: Any state (Alaska licensure not required).
- Inpatient Pharmacy Experience: Required.
- Experience: At least 2 years of clinical pharmacy experience or 1 year of post-graduate residency in a hospital setting.
- Clinical Pharmacy Skills: Experience in medication therapy management, direct patient care, and clinical pharmacy services.
