Job Title: Hospital Pharmacist (13-Week Assignment)
Location: Cincinnati, Ohio
Contract Length: 13 Weeks (with potential for permanent position)
Shift: 1st Shift, Monday-Friday + every 4th weekend
Hours: 10:00 AM - 7:00 PM

Job Summary:
We are seeking a skilled Hospital Pharmacist for a 13-week assignment at a healthcare facility in Cincinnati, Ohio. The position has the potential to convert to a permanent role, based on the candidate's performance and fit within the team. This position is to cover for a pharmacist who is out on sick leave.

Key Responsibilities:

• Perform order verification to ensure accuracy and compliance with protocols.
• Conduct medication reconciliation to confirm patient medication regimens.
• Provide consults for Warfarin and Vancomycin therapies.
• Adjust dosages based on renal function (renal dosing).
• Ensure IV compatibility for all prescribed medications.

Qualifications:

• Active and clear pharmacist license in the state of Ohio.
• Minimum of 2 years of pharmacist experience.
• Inpatient pharmacy experience required.
• Experience with WellSky EMR is preferred.
