Hospital Pharmacist - Elyria OH

We are hiring an experienced Inpatient Pharmacist for a full-time opportunity in Cleveland, OH. This role will be part of Cardiovascular Medicine, working directly with physicians and patients to ensure safe and effective medication use.

This position will focus on medication reconciliation by calling patients to review their current medications, discontinuing those they are no longer taking, managing refills, answering patient inquiries, and collaborating with physicians on medication-related concerns.

Schedule: Monday - Friday, 8:00 AM - 4:30 PM EST

Key Responsibilities:

• Review and verify medication orders for appropriate drug selection, dosage, and frequency.
• Identify and document adverse drug reactions.
• Accurately enter and update medication records.
• Adjust formulary defaults based on patient needs. Maintain and review controlled substance records, including daily and monthly inventories and DEA 222 forms.
• Check and update daily medication fill lists.
• Educate patients regarding their medications.
• Provide education to nursing and medical staff as requested.
• Follow aseptic techniques per infection control policies. Monitor and document laminar flow hood maintenance for compliance.
• Propose and implement at least one cost-saving strategy annually.
• Check medications prepared by pharmacy technicians before they are dispensed.

Requirements:

• Active Ohio State Pharmacist license in good standing with no negative history.
• Previous inpatient hospital experience required.
• Cardiovascular experience required.
