RPh on the go is searching for a Inpatient Hospital Pharmacist. The ideal candidate will have a year or more experience working in a hospital and will be able to work a flexible shift.
Key Responsibilities:

Medication Management:
- Review and verify medication orders for accuracy and appropriateness.
- Assess patient-specific factors to tailor medication therapy.
- Monitor patients for therapeutic efficacy and adverse effects.

Clinical Consultation:
- Collaborate with physicians, nurses, and other healthcare providers to optimize pharmacotherapy.
- Provide evidence-based recommendations for medication management and dosage adjustments.
- Participate in multidisciplinary rounds and patient care meetings.

Medication Orders:
- Prepare and dispense medications, including intravenous and high-risk drugs, following hospital protocols.
- Ensure accurate medication administration and labeling.

Patient Safety:
- Monitor and report adverse drug reactions and medication errors.
- Conduct medication therapy reviews to prevent drug interactions and contraindications.
- Participate in patient safety and quality improvement initiatives.

Education and Training:
- Educate healthcare staff and patients on medication use, including administration techniques and potential side effects.
- Provide training and guidance to pharmacy residents and interns.

Policy and Protocol Development:
- Assist in the development and implementation of medication use policies and clinical protocols.
- Ensure compliance with regulatory standards and best practices.

Medication Reconciliation:
- Perform medication reconciliation at patient transitions of care (e.g., admission, discharge, and transfer).
- Ensure accurate medication lists and address discrepancies.

Inventory Management:
- Oversee medication inventory, including procurement, storage, and distribution.
- Ensure compliance with inventory control procedures and regulatory requirements.

Research and Quality Improvement:
- Participate in clinical research and quality improvement projects.
- Contribute to the evaluation of medication use and therapeutic outcomes.

Required:
- Inpatient Hospital experience
- Arizona Pharmacist license clear and active
