Infusion Pharmacist - Contract to Hire - Chandler, Arizona

Job Description: As an Infusion Pharmacist, you will play a crucial role in providing specialized pharmaceutical care for patients requiring infusion therapy. Your expertise will ensure the safe and effective preparation and administration of intravenous medications, as well as provide critical guidance to healthcare providers and patients.

Key Responsibilities:

• Prepare and compound sterile intravenous medications, including chemotherapy, antibiotics, and other infusion therapies.
• Review and verify physician orders to ensure appropriate dosing and administration.
• Monitor patient responses to therapy and collaborate with healthcare providers to adjust treatment plans as needed.
• Educate patients and caregivers on proper medication administration, storage, and potential side effects.
• Ensure compliance with state and federal regulations, including USP <797> standards for sterile compounding.
• Maintain accurate records of all medications dispensed and therapy plans implemented.
• Participate in quality improvement initiatives and contribute to the development of best practices in infusion pharmacy.

Shift:

• Monday - Friday : 8-hour shifts within the hours of 8:00am -7:30pm
• Rotating "on call" weekends

Qualifications:

• AZ Pharmacist license
• 1-2 years in an infusion pharmacy setting
• CPR+ or Caretend experience
• Strong clinical and communication skills to work with patients and providers.
