Responsibilities:

Medication Therapy Management:

• Conduct comprehensive medication reviews for patients to identify and resolve medication-related issues.
• Collaborate with healthcare providers to develop and implement patient-specific treatment plans.

Clinical Support:

• Participate in interdisciplinary rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
• Provide drug information to healthcare providers and assist in the development of evidence-based guidelines.

Quality Improvement:

• Contribute to quality improvement initiatives by monitoring medication use processes and implementing improvements as needed.

Regulatory Compliance:

• Ensure compliance with state and federal regulations related to pharmacy practice.
• Stay current with new medications, treatment guidelines, and emerging trends in clinical pharmacy practice.

Qualifications:

• Clear and active pharmacist license in the state of Delaware
• Previous Inpatient Pharmacy experience preferred
