A hospital in Middleburg, OH, is seeking a local Inpatient Hospital Pharmacist with cardiovascular experience for a contract-to-perm opportunity. This role involves medication reconciliation, patient education, and collaboration with physicians in a cardiovascular medicine practice.

Duties:

• Conduct medication reconciliation over the phone with patients.
• Process medication refills and answer patient questions.
• Collaborate with physicians regarding medication concerns.
• Verify medication orders for appropriateness, dosage, and frequency.
• Identify and document adverse drug reactions.
• Maintain controlled substance records, including DEA 222 forms.
• Educate patients, nursing staff, and medical staff on medications.
• Ensure compliance with aseptic techniques and infection control policies.
• Implement cost-saving strategies and promote formulary adherence.

Schedule/Shift:

• Monday Friday: 8:00 AM 4:30 PM EST
• Local candidates only (No travel reimbursement provided)

Requirements:

• Active OH State Pharmacist License (in good standing, no negative history).
• Inpatient hospital experience is required.
• Previous cardiovascular experience is mandatory.
• Experience with medication reconciliation and patient counseling.
