Inpatient Hospital Pharmacist (Cardiovascular Experience) Middleburg, OH

We are seeking a licensed Inpatient Hospital Pharmacist with cardiovascular experience for a contract-to-hire opportunity in Middleburg, OH. This role is based in a physician practice specializing in Cardiovascular Medicine. The pharmacist will conduct medication reconciliation over the phone, handle medication refills, address patient inquiries, and collaborate with physicians on medication-related concerns.

Schedule: Monday - Friday, 8:00 AM 4:30 PM

Responsibilities:

• Review medication orders for appropriate drug selection, dosage, and frequency.

• Identify and document adverse drug reactions.

• Read and accurately enter medication orders.

• Adjust formulary defaults based on patient needs.

• Maintain and review controlled substance records, including daily/monthly inventories and DEA 222 forms.

• Review and check daily fill lists, updating medication lists as necessary.

• Educate patients on their medications.

• Provide medication education to nursing and medical staff as requested.

• Adhere to aseptic techniques per department infection control policies.

• Monitor laminar flow hood maintenance and performance records.

• Develop and implement at least one cost-saving strategy annually.

• Communicate with physicians to recommend formulary alternatives for non-formulary medication requests.

• Review and approve floor stock medication requests when necessary.

• Provide neonatal education to parents as applicable and verify medication dosages based on patient weight.

• Monitor lab results to prevent adverse drug reactions.

• Check medications prepared by pharmacy technicians before they are dispensed.

Requirements:

• Active Ohio State Pharmacist license in good standing with no negative history.

• Previous inpatient hospital experience required.

• Cardiovascular experience is required.

• Local candidates only.
