Position Details:

• Term: Contract-to-Perm
• Location: LOCAL ONLY (Candidates must reside within commuting distance of Middleburg, OH)
• Schedule: Monday to Friday, 8:00 am - 4:30 pm EST
• Focus: Cardiovascular Medicine and Medication Reconciliation

Key Responsibilities:

• Medication Reconciliation: Call patients to conduct medication reviews, verify current medications, and make necessary updates.
• Patient Support: Answer patient questions regarding medications and assist with medication refills.
• Collaboration: Work with physicians to resolve medication-related issues and concerns.
• Review Medication Orders: Ensure the appropriateness of drug selection, dosage, and frequency.
• Adverse Drug Reactions: Identify and document any adverse drug reactions.
• Controlled Substance Management: Maintain controlled substance records, including daily and monthly inventories, and DEA 222 forms.
• Patient and Staff Education: Educate patients and staff on medication usage and safe practices.
• Aseptic Technique: Follow infection control policies to maintain sterile environments and ensure safe pharmacy practices.
• Cost-Saving Strategies: Propose and implement cost-saving strategies within the department.
• Collaboration with Physicians: Request formulary medications when non-formulary drugs are requested.

Requirements:

• Active OH State Pharmacist License in good standing
• Inpatient hospital and cardiovascular experience required
• Strong ability to collaborate with both patients and medical staff
• Excellent communication skills for patient education and addressing medication concerns
