Inpatient Hospital Pharmacist-Philadelphia, PA
Position: Clinical Pharmacist
Hours: 32 hours/week
Schedule: On-site Monday, Wednesday, and Friday; choose either Tuesday or Thursday
Shift: M-F, 8:00 AM - 4:30 PM
Patient Rounding: 9:00 AM - 11:30 AM (availability required during this time)
Skills Required:
- Experience: Inpatient behavioral health PGY-2 or equivalent behavioral health experience
- Clinical Expertise: Ability to act as a clinical pharmacist during patient rounds
- Knowledge: Familiarity with The Joint Commission (TJC) standards
- Technical Skills: Proficient in Excel and comfortable with spreadsheets
- Laboratory Ordering: Ability to order lab work as needed
Job Responsibilities:
- Patient Rounds: Participate in morning patient rounds, providing support and knowledge on psychotropic and other medications, therapeutic drug monitoring (e.g., Lithium, Depakote), and metabolic monitoring.
- Liaison Role: Collaborate with LTC pharmacy (Polaris) to address and resolve issues, ensuring consistent and timely access to medications.
- Administrative Duties:
- Conduct data audits
- Serve as Chairperson for Pharmacy & Therapeutics (P&T) Committee
- Participate in Pain Management Stewardship initiatives
- Attend Medical Executive Committee (MEC) meetings
- Engage in Antimicrobial Stewardship programs
- TJC Preparation:
- Ensure familiarity with TJC standards
- Perform monthly medication room inspections in preparation for TJC evaluation expected around February.
