Job Title: Inpatient Hospital Pharmacist
Location: Prairie Du Sac, WI

Position Details:

• Contract Length: 13+ weeks
• Schedule: Monday to Friday with flexible hours (no weekends); shifts include 6:00 AM 4:30 PM and 10:30 AM 9:00 PM.
• On-Call: Possible requirement.
• Facility: 36-bed hospital (No Chemo, No 340B).

Requirements:

• Experience: 2+ years of recent inpatient hospital experience.
• Licensure: Clear and active Wisconsin pharmacist license.

Responsibilities:

1. Order Entry:

• Enter medication orders into Meditech system or perform quality control checks.
• Review orders for appropriateness, duplication, and safety (e.g., drug interactions, allergies).
• Clarify incomplete or unclear orders with the prescribing physician.
• Perform dosage calculations and adjustments for renal or hepatic impairments.
• Document therapeutic interchanges and clinical interventions.

2. Patient Care:

• Review computerized medication profiles and intervene for drug therapy problems.
• Provide drug information and perform drug monitoring consults.
• Manage TPN orders and therapeutic drug monitoring (e.g., warfarin, anticonvulsants, digoxin).
• Review labs and microbiology cultures, intervening as necessary.
• Document patient-related services, including 1st dose teaching, diabetes teaching, warfarin education, and discharge counseling.
