Long Term Care Pharmacist 13-Week Contract-to-Hire | Syracuse, NY

We are currently seeking a dedicated and experienced Staff Pharmacist to join a long-term care facility in Syracuse, NY on a 13-week contract-to-hire basis.

Position Overview:

In this role, you will provide high-quality pharmaceutical care to residents in a long-term care setting. This is an excellent opportunity to work in a supportive environment with potential for permanent placement following the contract period.

Key Responsibilities:

• Verify prescriptions for accuracy and compliance with state and federal regulations.
• Provide clinical consultations and medication therapy management.
• Prepare and dispense medications while maintaining detailed, accurate records.
• Collaborate with healthcare providers to ensure optimal patient outcomes.
• Monitor drug therapies for effectiveness and safety.
• Contribute to quality assurance and regulatory compliance initiatives.

Pharmacy Operating Hours:

• Monday - Friday: 8:00 AM - 9:00 PM
• Saturday: 8:00 AM - 6:00 PM
• Sunday: 8:00 AM - 5:00 PM

Qualifications:

• Long-term care (LTC) experience preferred.
• Minimum of 1 year of pharmacy experience.
• Active New York Pharmacist license required.
