Medication Reconciliation Pharmacist - Day Shift - New Jersey

RPh on the Go is actively seeking a Medication Reconciliation Pharmacist willing to work in Paramus, NJ. This role is a Monday-Friday, day shift position. The Medication Reconciliation Pharmacist will primarily work in the Emergency Department (ED), ensuring accurate medication history and reconciliation for patients. The ideal candidate will have previous ED experience or strong inpatient experience with excellent communication and collaboration skills.

Key Responsibilities:

• Conduct comprehensive medication reconciliation for patients upon admission, transfer, and discharge.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure medication accuracy.
• Identify and resolve medication discrepancies to prevent adverse drug events.
• Maintain thorough documentation and ensure compliance with hospital policies and regulatory standards.
• Provide medication-related education to patients and healthcare teams.
• Utilize electronic medical records (EMR) and other hospital systems to verify and update patient medication histories.

Requirements:

• Active and unrestricted Pharmacist license in the state of New Jersey.
• Previous Emergency Department experience is highly preferred.
• Strong inpatient pharmacy experience with proven collaboration across hospital departments.
• Exceptional communication and problem-solving skills.
• Proficiency in electronic medical records (EMR) and medication reconciliation processes.
