Pharmacist-Bethlehem, Pennsylvania
RPh on the go is searching for a Pharmacist with long term care experience. This is a order entry position.
Shift: Mid shift, M-F (3pm - 11:30pm), rotating every 4th weekend.
Key Responsibilities:

Medication Management:
- Review and assess medication orders for appropriateness, dosage, and potential drug interactions.
- Monitor resident medication regimens and adjust as needed to optimize therapeutic outcomes.
- Provide recommendations for medication therapy based on individual patient needs and conditions.

Patient Safety:
- Ensure the safe and effective use of medications by residents, including monitoring for and addressing adverse drug reactions and side effects.
- Develop and implement strategies to prevent medication errors and improve patient safety.
- Conduct medication reconciliation during admission, transfer, and discharge to avoid discrepancies and ensure continuity of care.

Regulatory Compliance:
- Maintain adherence to state and federal regulations, including the Drug Enforcement Administration (DEA) and other relevant agencies.
- Ensure compliance with facility policies and procedures related to medication management and documentation.
- Prepare for and participate in audits and inspections, addressing any findings or recommendations.

Collaborative Care:
- Work closely with physicians, nurses, and other healthcare professionals to develop and manage individualized care plans for residents.
- Participate in care team meetings to discuss medication-related issues and contribute to patient care planning.
- Provide educational sessions and training for facility staff on medication management and pharmaceutical care.

Pharmacy Operations:
- Oversee the procurement, storage, and dispensing of medications in accordance with legal and safety standards.
- Manage inventory, including ordering, stocking, and ensuring proper handling and storage of medications.
- Maintain accurate records of medication administration, distribution, and inventory.

Patient and Family Education:
- Educate residents and their families about medication regimens, including proper usage, potential side effects, and drug interactions.
Responsibilities
- Fills prescription items, as permitted by the State Board of Pharmacy, packaging medications for dispensing and for delivery, meeting the accuracy and productivity standards of the pharmacy
- Labels products appropriately, including cautionary or auxiliary labels
- Participates in the periodic inventory of the pharmacy
Required:
- Pharmacy experience.
- LTC experience a plus.
- Pennsylvania pharmacy license.
