Duration: 12 weeks, with the potential for direct hire
Environment: Long-Term Care (LTC) Mail Order, Managed Care
Schedule: Monday to Friday, 9:00 AM - 6:00 PM, rotating weekends, rotating on-call weekend schedule.
Job Description:
We are looking for an experienced Pharmacist to join our team in a Long-Term Care (LTC) mail-order pharmacy environment. This is a 12-week contract position with the possibility of direct hire. The ideal candidate will have a strong background in pharmacy practice, including experience in LTC and managed care settings. This role involves dispensing medications, providing medication therapy management, collaborating with healthcare teams, and ensuring high-quality patient care for residents in LTC facilities. The position also includes rotating weekend and on-call responsibilities.
Key Responsibilities:
- Medication Dispensing: Accurately dispense medications to LTC patients, ensuring correct dosages and instructions.
- Patient Counseling: Provide medication counseling to LTC residents, caregivers, and healthcare providers as needed.
- Medication Therapy Management: Monitor and manage medication therapies, identifying potential drug interactions and recommending changes when appropriate.
- Collaborate with Healthcare Providers: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated care for residents.
- Pharmacy Operations: Oversee pharmacy operations including medication preparation, inventory control, and ensuring proper storage of medications.
- Regulatory Compliance: Ensure compliance with all federal, state, and local laws and regulations governing pharmacy practice, especially in the LTC and managed care environments.
- Customer Service: Provide exceptional service to patients, families, and healthcare providers, addressing concerns and medication-related inquiries.
- On-Call and Weekend Schedule: Participate in a rotating weekend and on-call schedule, ensuring continuous pharmacy support.
Required Qualifications:
- Active Montana Pharmacy License.
- Pharmacy Experience: Proven experience working in a pharmacy setting.
- LTC Experience: Experience working in Long-Term Care pharmacy is highly preferred.
- Managed Care Experience: Experience working in a managed care setting is a plus.
- Attention to Detail: Strong ability to manage complex medication regimens for LTC residents.
- Communication Skills: Excellent verbal and written communication skills, with the ability to collaborate effectively with healthcare teams.
