RPh on the Go is actively seeking a Pharmacist to work in a long-term care (LTC) facility. The ideal candidate will have at least one year of pharmacy experience, with LTC experience being a plus. This is a long-term opportunity for a dedicated professional with a focus on patient care and medication management.

Shift Details:

• 2nd Shift
• Hours: 2:00 PM - 11:00 PM
• Possible weekend shifts

Key Responsibilities:

• Oversee the preparation, dispensing, and documentation of medications for long-term care patients.
• Ensure accurate and safe medication distribution in accordance with prescriptions and facility policies.
• Conduct medication reviews, monitor patient drug therapies, and provide appropriate recommendations to healthcare teams.
• Counsel healthcare providers and patients on medication usage, potential side effects, and interactions.
• Collaborate with the interdisciplinary team to optimize therapeutic outcomes for patients in long-term care settings.
• Maintain compliance with all state and federal regulations governing pharmacy operations.

Required Qualifications:

• Minimum of 1 year of pharmacy experience.
• Long-term care experience is a plus.
• Active West Virginia pharmacist license.
• Strong attention to detail and ability to work in a fast-paced environment.
• Excellent communication and interpersonal skills.
