Pharmacist- Long Term Care- Ventura, CA

RPh on the Go is searching for a pharmacist join our team in Ventura, California. This is an 12 week contract position with direct hire potential and we are actively interviewing to start ASAP.

Schedule:
M - F: 12pm - 8:30pm, Sat: 9am -5:30pm.

Responsibilities:

• Dispense medications accurately and efficiently.
• Review and verify physician orders for accuracy and appropriateness.
• Provide drug information and education to healthcare providers and patients.
• Conduct medication therapy management and review patient medication regimens.
• Monitor for potential drug interactions and adverse reactions.
• Maintain accurate records and documentation.
• Ensure compliance with all applicable laws, regulations, and facility policies.

Required:

• Pharmacy experience.
• LTC experience a plus
• California pharmacy license (clear and active)
