RPh on the go is searching for Pharmacist. The ideal candidate will have a year or more LTC /retail experience.

Shift: M-F, Mid Shift (11am - 7:30pm). 32 hours/week

Key Responsibilities:

  1. Medication Management:

    • Review and analyze patient medication profiles to ensure appropriate, safe, and effective medication use.
    • Monitor and evaluate medication regimens for therapeutic efficacy, potential side effects, and drug interactions.
    • Recommend adjustments to medication therapy as needed, based on patient response and clinical guidelines.
  2. Patient Care:

    • Conduct medication therapy management (MTM) and medication reconciliation for new admissions and periodic reviews.
    • Provide drug information and education to patients, families, and caregivers on proper medication use and potential side effects.
    • Collaborate with healthcare teams, including physicians, nurses, and other staff, to optimize patient care plans.
  3. Compliance and Documentation:

    • Ensure compliance with federal, state, and local regulations regarding medication management and documentation.
    • Maintain accurate and up-to-date records of medication orders, changes, and patient responses.
    • Assist with audits and inspections to ensure adherence to regulatory standards.
  4. Quality Assurance:

    • Implement and monitor quality improvement initiatives related to medication use and patient safety.
    • Develop and review policies and procedures related to medication management and pharmacy services.
  5. Education and Training:

    • Provide training and support to facility staff on medication-related topics, including new drug therapies and best practices.
    • Stay current with advancements in pharmacotherapy and long-term care practices through continuous education and professional development.
  6. Pharmacy Operations:

    • Oversee the procurement, storage, and dispensing of medications within the facility.
    • Ensure proper handling and disposal of medications, including controlled substances.
    • Coordinate with pharmacy vendors and suppliers to ensure timely and accurate medication

Required:

  • Pharmacy experience.
  • LTC experience a plus.
  • Washington Pharmacy license
