Specialty Clinical Pharmacist - Rochester, NY
RPH on the go is seeking a dedicated Clinical Pharmacy Specialist for a Level 1 Trauma Center. This position focuses on optimizing safe, effective, and cost-efficient drug therapy while contributing to clinical innovation, mentoring, and interdisciplinary collaboration. The role requires specialized expertise and offers the opportunity to influence best practices in pharmacotherapy within a dynamic healthcare environment.
Clinical Pharmacy Practice:
- Provide comprehensive clinical pharmacy services for trauma patients and other specialized populations.
- Participate in inter-disciplinary rounds, providing consultative input on drug therapy.
- Manage clinical pharmacy programs, monitor patient progress, and recommend therapy adjustments.
- Deliver patient education, discharge counseling, and supervise drug distribution as required.
- Contribute to developing guidelines, critical pathways, and disease management programs.
- Mentor pharmacy colleagues and foster professional growth.
- Conduct QA and Medication Use Evaluations (MUE), and support research initiatives.
Training:
- Serve as a preceptor for pharmacy residents and students, offering IPPE and APPE rotations.
- Provide educational programs, lectures, and case studies to pharmacy and medical staff.
- Offer continuing education to peers at local, regional, and national levels.
Problem Solving:
- Proactively resolve patient care issues, collaborating with staff for effective outcomes.
- Participate in interdisciplinary teams, revising policy, procedures, or treatment guidelines.
Service:
- Active involvement in local, regional, and national pharmacy organizations.
- Contribute to education and research presentations, continuing professional education, and hospital committees.
Schedule/Shift:
- Full-time, variable shifts with flexibility for operational needs.
Requirements:
- Education: Doctor of Pharmacy (PharmD).
- Licensure: New York State Pharmacy License.
- Experience:
- Postgraduate Year-1 (PGY-1) and Postgraduate Year-2 (PGY-2) residency training in a specialty area or equivalent clinical experience.
- 3-5 years of practice preferred for Level II designation.
- Certifications: Must be board certified in a relevant specialty area within 2 years of hire.
- Strong communication, leadership, and problem-solving skills.
