Staff Pharmacist Contract to Hire - Louisville, Kentucky

RPh on the Go is actively seeking a dedicated and compassionate Staff Pharmacist to join our team and work at a Long-Term Care (LTC) facility in Louisville, Kentucky. As a Staff Pharmacist, you will play a crucial role in ensuring that patients in the LTC facility receive optimal pharmaceutical care.

Key Responsibilities:

• Prepare, dispense, and manage medications for patients in the Long-Term Care setting.
• Review and interpret physician orders, ensuring accurate medication administration.
• Provide clinical consultation and support to healthcare staff, patients, and families.
• Conduct medication regimen reviews to identify potential drug interactions and optimize patient outcomes.
• Maintain compliance with all state and federal regulations, including but not limited to Board of Pharmacy and CMS guidelines.
• Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
• Monitor patient responses to therapy, making adjustments as necessary in coordination with healthcare providers.
• Ensure proper storage and handling of all medications in the facility.
• Document all activities in accordance with facility policies and procedures.

Shift:

• Monday - Friday: 8:00am - 7:30pm
• Rotating weekends
• 8 hour varied shifts

Qualifications:

• IN Pharmacist license
• Pharmacy experience.
• LTC experience a plus.
• Experience with DocuTrack/Frameworks a plus
