Staff Pharmacist Day Shift - Wilkes-Barre, Pennsylvania

RPh on the Go is seeking a dedicated and experienced Staff Pharmacist to join our team at a Long Term Care facility in Wilkes-Barre, Pennsylvania. This is a full-time, day shift position that offers a unique opportunity to make a significant impact on the health and well-being of residents in a supportive and collaborative environment.

Key Responsibilities:

• Provide comprehensive pharmaceutical care to residents in a Long Term Care setting.
• Review and verify medication orders for accuracy and appropriateness.
• Collaborate with healthcare providers, nurses, and other staff to ensure optimal patient outcomes.
• Monitor and manage medication therapy, including adjustments based on patient-specific factors.
• Ensure compliance with state and federal regulations, as well as facility policies.
• Maintain accurate records and documentation in accordance with regulatory standards.
• Provide patient education and counseling as needed.

Shift:

• Monday - Friday: 8:00am - 5:30pm
• Rotating weekends.

Qualifications:

• Pharmacy experience
• LTC experience a plus
• PA pharmacy license
