Staff Pharmacist - Long Term Care - Louisville, Kentucky

RPh on the Go is seeking a dedicated and experienced Staff Pharmacist to join our team in a Long Term Care facility located in Louisville, Kentucky. This is an excellent opportunity for a pharmacist who is passionate about providing exceptional care to patients in a long-term care setting.

Key Responsibilities:

• Accurately prepare and dispense medications according to physician orders.
• Provide medication counseling and support to healthcare staff and patients.
• Ensure compliance with all state and federal regulations governing pharmacy practice.
• Conduct medication reviews to ensure safety, efficacy, and appropriateness.
• Collaborate with healthcare professionals to optimize patient outcomes.
• Manage inventory levels and ensure timely ordering of medications and supplies.
• Maintain accurate records and documentation in accordance with facility policies.

Shift:

• Monday - Friday: 8:00am - 7:30pm
• 8 hour varied shift

Qualifications:

• IN Pharmacist license
• Pharmacy experience.
• LTC experience a plus.
• Experience with DocuTrack/Frameworks a plus
