Staff Pharmacist - Louisville, Kentucky

RPh on the Go is actively seeking a dedicated and experienced Staff Pharmacist to join our team and work at a well-established Long Term Care (LTC) facility in Louisville, Kentucky. This is an excellent opportunity for a pharmacist with a passion for patient care and a desire to make a difference in a specialized healthcare setting.

Key Responsibilities:

• Dispense medications accurately and efficiently, ensuring compliance with state and federal regulations.
• Review and interpret physician orders, ensuring appropriate drug therapy for patients.
• Provide drug information and education to healthcare staff, patients, and their families.
• Manage inventory, including ordering, stocking, and maintaining proper storage of medications.
• Collaborate with healthcare professionals to optimize medication therapy for residents.
• Monitor patient outcomes, including therapeutic drug monitoring and assessing potential drug interactions.
• Participate in the development and implementation of pharmacy policies and procedures.
• Ensure the highest standards of care and safety are maintained at all times.

Shift:

• Monday - Friday: 8:00am - 7:30pm
• Rotating weekends
• 8 hour varied shift

Qualifications:

• IN Pharmacist license
• Pharmacy experience.
• LTC experience a plus.
• Experience with DocuTrack/Frameworks a plus
