Job Description: Inpatient Pharmacist

Environment: Inpatient Hospital
Shift: Overnights (9 PM - 7 AM, 7 on 7 off)
Computer System: Cerner

Position Summary: We are seeking a dedicated Inpatient Pharmacist to join our team, providing comprehensive pharmaceutical care in a fast-paced hospital environment. The ideal candidate will be responsible for ensuring safe and effective medication management for patients during overnight shifts.

Key Responsibilities:

• Review and verify medication orders for accuracy, appropriateness, and safety.
• Collaborate with healthcare teams to optimize pharmacotherapy and improve patient outcomes.
• Provide drug information and support to medical staff and patients.
• Monitor patient responses to medication therapy and adjust regimens as necessary.
• Participate in the management of medication therapy, including compounding and dispensing medications.
• Conduct medication reconciliation and participate in discharge planning.
• Ensure compliance with hospital policies, state, and federal regulations regarding pharmacy practice.
• Maintain accurate documentation within the Cerner system.
• Participate in quality assurance initiatives and continuous improvement projects.

Qualifications:

• Current Illinois pharmacy license.
• Previous experience in an inpatient pharmacy setting.
• Strong knowledge of pharmacotherapy and medication management.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Familiarity with Cerner or similar electronic health record systems is a plus.

Shift Details:

• Overnight shifts from 9 PM to 7 AM.
• 7 days on, 7 days off schedule.
