Staff Pharmacist-Weslaco- Texas
RPh on the go is searching for a Pharmacist. The ideal candidate will have a year or more experience working in a long term facility
Shift: M-F, day/evening rotation (8am - 5pm, 11am - 7pm), rotating every other weekend.
Key Responsibilities:

Medication Management:
- Review and verify medication orders for accuracy and appropriateness.
- Monitor residents medication regimens to ensure efficacy, safety, and adherence.
- Assess and manage medication-related issues and make necessary adjustments.

Clinical Consultation:
- Collaborate with physicians, nurses, and other healthcare providers to optimize pharmacotherapy for residents.
- Provide recommendations on medication therapy, dosage adjustments, and alternative treatments.
- Participate in interdisciplinary care meetings and care plan discussions.

Medication Orders:
- Ensure accurate preparation and dispensing of medications, including oral, topical, and injectable forms.
- Oversee medication administration practices to ensure compliance with prescribed therapy.

Patient Safety:
- Monitor residents for adverse drug reactions and medication errors.
- Conduct medication therapy reviews to identify and prevent drug interactions and contraindications.
- Develop and implement strategies to enhance medication safety and efficacy.

Medication Reconciliation:
- Perform medication reconciliation during transitions of care, such as admissions, discharges, and transfers.
- Ensure accurate and up-to-date medication lists for all residents.

Education and Training:
- Educate healthcare staff and residents on proper medication use, administration techniques, and potential side effects.
- Provide training to new staff and ongoing education to existing staff regarding medication-related topics.

Policy and Protocol Development:
- Contribute to the development and implementation of medication use policies and clinical guidelines.
- Ensure adherence to regulatory requirements and best practices in medication management.

Quality Improvement:
- Participate in quality improvement initiatives to enhance medication management and resident care.
- Monitor and evaluate medication-related outcomes and implement changes to improve care quality.

Inventory Management:
- Oversee medication inventory, including procurement, storage, and distribution.
- Ensure compliance with inventory control procedures and regulatory standards.

Documentation and Reporting:
- Maintain accurate and detailed documentation of medication-related activities and interventions.
- Report any medication errors, adverse events, or safety concerns in accordance with facility policies.
Required:
- Pharmacy experience.
- LTC experience a plus
- Texas pharmacy license.
