Clinical Pharmacist in Baldwinsville, NY

A direct hire opportunity is available for a Home Infusion Staff Pharmacist in the Baldwinsville, NY area, supporting patients in the Syracuse region. This role offers a Monday through Thursday schedule, from 8:00 AM to 5:00 PM, providing an excellent work-life balance.

Key Responsibilities:

• Review and verify infusion orders and patient therapies.
• Participate actively in clinical rounds to optimize patient care.
• Perform sterile compounding and comprehensive review of Total Parenteral Nutrition (TPN).
• Maintain accurate patient records and individualized care plans.
• Ensure compliance with USP standards and cleanroom protocols.
• Train and support pharmacy staff in technical and clinical practices.
• Drive operational improvements to enhance service quality and safety.

Desired Experience and Qualifications:

• Current Registered Pharmacist license in New York State is required.
• A minimum of two years recent acute care pharmacy experience or at least one year in home infusion therapy is preferred.
• Expertise in intravenous admixture services and intravenous therapy.
• Proven ability to evaluate and monitor drug therapies in collaboration with multidisciplinary healthcare teams, tailor treatment plans to patient clinical conditions, and ensure optimal outcomes.
