Home Infusion Clinical Pharmacist | Brookfield, Wisconsin

This position offers the opportunity to apply your clinical pharmacy expertise in home infusion services within a supportive healthcare setting. Focused on clinical pharmacology and sterile compounding, this role requires recent IV experience and active licensure to ensure the highest standards in patient care are maintained.

Location: Brookfield, WI
Job Type: Contract to Hire

Schedule:

• Monday to Friday, 8-hour shifts between 7:30 AM and 6:00 PM
• On call every 4th weekend
• One night on call per week

Qualifications and Desired Experience:

• Active and clear Wisconsin pharmacist license
• Minimum of 2 years recent home infusion experience preferred
• At least 2 years of recent sterile compounding experience required
• Current CPR certification preferred
• Proven ability to work in compliance with clinical and safety guidelines

Key Responsibilities:

• Accurately verify IV medication orders to ensure patient safety and treatment efficacy
• Prepare and mix IV medications in the clean room as needed
• Perform other clinical pharmacy duties supportive of home infusion services
• Collaborate with healthcare teams to optimize patient outcomes through medication management
