Clinical Pharmacist - Cicero, New York

A direct hire opportunity is available for a Home Infusion Staff Pharmacist specializing in clinical pharmacy and medication therapy management. This role offers a Monday through Thursday schedule, from 8:00 AM to 5:00 PM, providing a consistent workweek in a home infusion setting.

Schedule:

• Monday - Thursday
• 8:00 AM - 5:00 PM

Responsibilities:

• Review and verify infusion orders and individualized patient therapies to ensure accuracy and safety
• Participate in clinical rounds to collaborate with healthcare teams on patient care plans
• Perform sterile compounding and thorough review of total parenteral nutrition (TPN) formulations
• Maintain detailed patient records and develop comprehensive care plans
• Ensure compliance with United States Pharmacopeia (USP) standards and cleanroom protocols
• Provide training and support to pharmacy and clinical staff to uphold quality standards
• Drive ongoing operational improvements in home infusion pharmacy services

Qualifications:

• Licensed Registered Pharmacist in the State of New York
• Minimum two years of recent acute care experience or at least one year of recent home infusion therapy experience preferred
• Strong expertise in intravenous admixture services and intravenous therapy
• Skilled in evaluating and monitoring drug therapy tailored to home care patients, working collaboratively with physicians, nurses, and interdisciplinary healthcare professionals
• Familiarity with CareTend pharmacy systems is a plus
