Home Infusion Clinical Pharmacist in Dallas, TX

A Home Infusion Team located in North Dallas, TX is seeking a direct hire, full-time pharmacist with strong hands-on sterile compounding experience. This role is ideal for a pharmacist who is highly skilled with IV preparation and compounding practices, working a consistent day shift schedule of 40 hours per week.

Responsibilities include:

• Reviewing physician-issued prescriptions for accuracy and determining appropriate formulas and ingredients
• Preparing, filling, and compounding medications following standard formulas, weighing, measuring, and mixing ingredients precisely
• Entering patient, medication, and cost data to maintain thorough pharmacy files and inventory records
• Providing drug usage reports to pharmaceutical partners as required
• Performing PMP reporting to comply with state regulations
• Organizing pharmacy activities to meet patient needs according to physician directions and best practices
• Participating in continuing education to enhance pharmacy services
• Assisting leadership with policy development, enforcement, and updates
• Contributing to quality assurance programs to monitor patient care safety and documentation appropriateness
• Supporting adherence to all State and Federal pharmacy laws and regulations

Qualifications required:

• Graduate of an accredited school of pharmacy
• Possession of a valid Texas pharmacist license in good standing (additional state licenses a plus)
• Proven USP 797 sterile compounding experience
• Extensive infusion and IV medication experience
• Intrathecal experience is considered a plus
