Clinical Pharmacist in Fairport, NY

A direct hire opportunity is available for a Home Infusion Staff Pharmacist in the Rochester, NY area. This role offers a Monday through Thursday schedule, 8:00 AM to 5:00 PM, working within a CareTend system environment.

Key Responsibilities:

• Review infusion orders and patient therapies to ensure accuracy and safety.
• Participate in clinical rounds to support patient care decisions.
• Perform sterile compounding and conduct total parenteral nutrition (TPN) reviews.
• Maintain detailed patient records and individualized care plans.
• Ensure compliance with USP standards and cleanroom protocols.
• Provide training and ongoing support to pharmacy staff.
• Drive operational improvements to enhance service delivery.

Qualifications:

• Current licensure as a Registered Pharmacist in New York State.
• Minimum of two years recent experience in an acute care setting or at least one year in home infusion therapy preferred.
• Strong knowledge and hands-on experience in intravenous admixture services and intravenous therapy.
• Ability to evaluate and monitor drug therapy tailored to the clinical needs of home care patients, collaborating effectively with physicians, nurses, and other healthcare professionals.
