Home Infusion Clinical Pharmacist - Frisco, Texas

This opportunity is ideal for a clinical pharmacist with specialized experience in home infusion and sterile compounding. Over a 13-week contract, you will play a critical role in patient-centered care by evaluating eligibility for home infusion therapy and developing tailored medication plans grounded in clinical assessments and pharmacokinetics.

Responsibilities include:

• Evaluating patients for home-based infusion therapy eligibility
• Preparing, verifying, and dispensing sterile medications according to USP standards and regulatory requirements
• Educating patients, caregivers, and healthcare providers on therapy regimens and infusion administration
• Collaborating with healthcare teams to ensure continuity of care and monitor outcomes
• Managing infusion devices and maintaining quality assurance and safety protocols
• Providing drug information and supporting regulatory compliance and medication safety initiatives

Minimum qualifications:

• Doctor of Pharmacy (PharmD) degree or graduate of an accredited School of Pharmacy
• Active, unrestricted Texas Pharmacist license
• One or more of the following:
  • At least 1 year of sterile IV compounding experience
  • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM)
  • Completion of an ASHP-accredited Pharmacy Residency or an Advanced Pharmacy Practice Experience (APPE)
  • Equivalent relevant pharmacy experience

Required skills:

• Strong clinical knowledge of home infusion pharmacy practices
• Proficient in interpreting laboratory values and applying pharmacokinetic principles
• Excellent communication, patient counseling, and customer service skills

• Monday to Friday, 8:30 AM 5:00 PM
• Monday to Friday, 7:30 AM 3:30 PM
