A clinical pharmacist role specializing in sterile compounding for home infusion therapy is available for a 13-week contract assignment in Garland, TX. This position requires a skilled pharmacist to evaluate patient eligibility, develop individualized medication plans, and prepare sterile medications adhering to USP and regulatory standards.

Responsibilities:

  • Assess patients for home infusion therapy and determine eligibility for home-based care.
  • Develop, monitor, and adjust medication therapy plans using clinical data and pharmacokinetic principles.
  • Prepare, verify, and dispense sterile medications following USP standards and regulatory guidelines.
  • Educate patients, caregivers, and healthcare providers on infusion therapy and proper medication use.
  • Collaborate with healthcare providers to ensure continuity of care and monitor patient outcomes.
  • Manage infusion pumps and administration equipment, maintaining quality assurance and safety.
  • Provide drug information, maintain documentation, and support compliance and medication safety initiatives.

Qualifications & Desired Experience:

  • Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
  • Active, unrestricted Pharmacist license in Texas.
  • Minimum of one of the following:
    At least 1 year of sterile IV compounding experience, or
    Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM), or
    Completion of an ASHP-accredited Pharmacy Residency, or
    Completion of an Advanced Pharmacy Practice Experience (APPE), or
    Equivalent pharmacy experience.
  • Strong clinical knowledge of home infusion pharmacy practices.
  • Proficient in interpreting laboratory values and applying pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.

Schedule:

  • Monday to Friday, 8:30 AM to 5:00 PM OR
  • Monday to Friday, 7:30 AM to 3:30 PM
