Home Infusion Clinical Pharmacist - Irving, TX
A home infusion pharmacy team located in North Dallas, TX is seeking a dedicated, full-time pharmacist for a direct hire position. This role is ideal for a clinical pharmacist with extensive experience in sterile compounding and IV infusion. The position offers a hands-on opportunity to prepare and compound medications that support patient care in home-based settings.
Schedule:
- Full Time (40 hours/week)
- Day Shift
Key Responsibilities:
- Review and verify prescriptions for accuracy, determining appropriate formulas and ingredients.
- Prepare, fill, and compound sterile medications using precise measuring and mixing techniques in compliance with USP 797 standards.
- Maintain accurate pharmacy records, including patient information, medication details, and inventory management.
- Provide drug usage reports to pharmaceutical partners as needed.
- Perform Prescription Monitoring Program (PMP) reporting in accordance with state regulations.
- Plan and organize pharmacy activities to meet patient and physician needs following best practices.
- Participate in ongoing continuing education to enhance pharmacy service quality.
- Enforce and update pharmacy policies and procedures.
- Support quality assurance initiatives to ensure safe and effective drug therapy.
- Assist leadership in ensuring compliance with all applicable state and federal pharmacy laws and regulations.
Qualifications:
- Graduate from an accredited school of pharmacy.
- Active Texas pharmacist license in good standing; additional state licenses are advantageous.
- Required experience in USP 797 sterile compounding.
- Required infusion/IV experience.
- Intrathecal experience is a plus.
