Clinical Pharmacist - Kuna, ID

An opportunity is available for a licensed Clinical Pharmacist to contribute expertise in an inpatient rehabilitation hospital setting. This contract position offers a 13-week assignment with the potential for extension based on facility needs. The role involves working with a dynamic healthcare team to ensure safe and effective medication use for patients recovering from various conditions.

Location: Kuna, ID
Assignment Length: 13 weeks, possibility for extension
Setting: Inpatient Rehabilitation Hospital
Hours:

• Monday to Friday, 8:30 AM to 7:00 PM
• Rotating weekends
• 8-hour shifts
  EMR System: Ace-It (Cerner-modified application)

Key Responsibilities:

• Review and process medication orders for accuracy, appropriate dosing, and potential drug interactions
• Conduct medication reconciliation by reviewing patients' medication histories and identifying discrepancies
• Educate patients regarding medication usage, dosing instructions, and potential side effects
• Collaborate closely with physicians and nursing staff to address medication-related concerns and support treatment planning
• Identify and mitigate medication-related risks to enhance patient safety
• Support daily pharmacy operations including medication dispensing and workflow management
• Maintain accurate and compliant documentation using the Ace-It EMR system
• Adhere to pharmacy regulations, hospital policies, and safety standards

Qualifications:

• Current and valid pharmacist license (preferably in Illinois) in good standing
• At least one year of hospital pharmacy experience is preferred
• Strong clinical knowledge with a focus on inpatient rehabilitation medication management
• Proficiency in EMR systems is an advantage
