A Home Infusion Staff Pharmacist position is available in Syracuse, NY, offering a Monday through Thursday schedule from 8:00 AM to 5:00 PM. This direct hire opportunity involves working with the CareTend system to provide specialized pharmacy services in a home infusion setting.

Responsibilities include reviewing infusion orders and patient therapies to ensure accuracy and efficacy, participating in clinical rounds to collaborate with healthcare teams, and performing sterile compounding and total parenteral nutrition (TPN) reviews. Maintaining detailed patient records and care plans is essential, along with ensuring compliance with USP standards and cleanroom protocols. Qualifications: Current licensure as a Registered Pharmacist in the State of New York
Minimum of two years of recent experience in an acute care setting or at least one year of recent home infusion therapy experience preferred
Strong knowledge and experience in intravenous admixture services and intravenous therapy
Ability to evaluate and monitor drug therapy in collaboration with physicians, nurses, and other healthcare professionals, focusing on the total clinical needs of home care patients
