Clinical Pharmacist - Nampa, Idaho

A licensed Clinical Pharmacist is needed for a contract assignment at an inpatient rehabilitation hospital in Nampa, ID. This opportunity spans 13 weeks with the potential for extension based on facility needs. Work within a dynamic hospital setting utilizing the Ace-It EMR system, a Cerner-modified application, while providing critical pharmaceutical care to support patient recovery and safety.

Location:

• Nampa, ID

Schedule/Hours:

• Monday to Friday, 8:30 AM - 7:00 PM
• Rotating weekends
• 8-hour shifts

Key Responsibilities:

• Review and process medication orders for accuracy, dosing appropriateness, and interaction risks
• Conduct medication reconciliation by reviewing histories and identifying discrepancies
• Educate patients about medication usage, dosing schedules, and possible side effects
• Collaborate with physicians and nursing staff to resolve medication-related issues and aid treatment decisions
• Monitor and promote medication safety by identifying risks and implementing preventative measures
• Assist with daily pharmacy operations and medication dispensing within the hospital
• Maintain precise documentation using the Ace-It EMR system
• Ensure adherence to pharmacy regulations, hospital policies, and safety standards

Qualifications:

• Active Pharmacist license in good standing
• Minimum one year of hospital pharmacy experience preferred
• Proficient in clinical pharmacology and medication management in inpatient settings
