Clinical Pharmacist - Penfield, NY

A direct hire opportunity is available for a Clinical Pharmacist specializing in home infusion services in the Rochester, NY area. This role offers a Monday through Thursday schedule from 8:00 AM to 5:00 PM, providing an excellent work-life balance while engaging in meaningful patient care.

Responsibilities include reviewing infusion orders and patient therapies to ensure optimal outcomes. The pharmacist will actively participate in clinical rounds, perform sterile compounding and total parenteral nutrition (TPN) reviews, and maintain comprehensive patient records and care plans. Ensuring compliance with USP standards and cleanroom protocols is critical, along with training and supporting pharmacy staff. This position also focuses on driving operational improvements within the home infusion practice. The position utilizes the CareTend system to manage clinical workflows effectively.

Qualifications:

• Current licensure as a Registered Pharmacist in New York State is required.
• A minimum of two years recent experience in an acute care setting or at least one year of home infusion therapy experience is preferred.
• Strong knowledge and experience with intravenous admixture services and intravenous therapy are essential.
• Ability to assess and monitor drug therapy in home care patients, collaborating closely with physicians, nurses, and other healthcare professionals.
