This 13-week contract opportunity offers a specialized role as a Sterile Compounding Pharmacist focused on home infusion therapy in Plano, TX. The position supports patient-centered care through evaluation, medication management, and education within a clinical pharmacy setting specializing in home infusion.

Schedule:

  • Monday to Friday, 8:30 AM 5:00 PM
    OR
  • Monday to Friday, 7:30 AM 3:30 PM

Responsibilities:

  • Assess patient eligibility for home infusion therapy and coordinate home-based care plans.
  • Develop, monitor, and adjust individualized medication therapy using clinical assessments, lab values, and pharmacokinetic principles.
  • Prepare, verify, and dispense sterile medications, adhering to USP standards and regulatory requirements.
  • Educate patients, caregivers, and healthcare providers on medication use, infusion administration, and safety protocols.
  • Collaborate with healthcare teams to ensure continuity of care and monitor patient outcomes.
  • Manage infusion pumps and equipment, maintaining quality assurance and safety standards.
  • Provide drug information, maintain accurate documentation, and support compliance and medication safety initiatives.

Qualifications:

  • Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
  • Active, unrestricted Texas Pharmacist license.
  • At least one of the following:
    • Minimum 1 year of sterile IV compounding experience,
    • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
    • Completion of an ASHP-accredited Pharmacy Residency,
    • Completion of an Advanced Pharmacy Practice Experience (APPE), or
    • Equivalent pharmacy experience.

Required Skills:

  • Strong clinical expertise in home infusion pharmacy practices.
  • Ability to interpret lab values and apply pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.
