Home Infusion Clinical Pharmacist | Plano, Texas
This 13-week contract opportunity offers a specialized role as a Sterile Compounding Pharmacist focused on home infusion therapy in Plano, TX. The position supports patient-centered care through evaluation, medication management, and education within a clinical pharmacy setting specializing in home infusion.
Schedule:
- Monday to Friday, 8:30 AM 5:00 PM
OR - Monday to Friday, 7:30 AM 3:30 PM
Responsibilities:
- Assess patient eligibility for home infusion therapy and coordinate home-based care plans.
- Develop, monitor, and adjust individualized medication therapy using clinical assessments, lab values, and pharmacokinetic principles.
- Prepare, verify, and dispense sterile medications, adhering to USP standards and regulatory requirements.
- Educate patients, caregivers, and healthcare providers on medication use, infusion administration, and safety protocols.
- Collaborate with healthcare teams to ensure continuity of care and monitor patient outcomes.
- Manage infusion pumps and equipment, maintaining quality assurance and safety standards.
- Provide drug information, maintain accurate documentation, and support compliance and medication safety initiatives.
Qualifications:
- Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
- Active, unrestricted Texas Pharmacist license.
- At least one of the following:
- Minimum 1 year of sterile IV compounding experience,
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
- Completion of an ASHP-accredited Pharmacy Residency,
- Completion of an Advanced Pharmacy Practice Experience (APPE), or
- Equivalent pharmacy experience.
Required Skills:
- Strong clinical expertise in home infusion pharmacy practices.
- Ability to interpret lab values and apply pharmacokinetic principles.
- Excellent communication, patient counseling, and customer service skills.
