Home Infusion Clinical Pharmacist - Plano, Texas
A Home Infusion Team located in North Dallas, TX is seeking a dedicated, full-time pharmacist to join their direct hire team. This position offers a hands-on role in sterile compounding with a strong emphasis on intravenous (IV) medication preparation and home infusion pharmacy services. Candidates with extensive IV experience will find this role especially rewarding.
Schedule:
- Full Time, 40 hours per week
- Day Shift
Key Responsibilities:
- Review and verify prescriptions issued by authorized prescribers to ensure accuracy and determine required formulas and ingredients.
- Prepare, fill, and compound medications using standard formulas and aseptic techniques, including weighing, measuring, and mixing ingredients.
- Enter and maintain accurate pharmacy records, including patient details, prescribed medications, and inventory data.
- Generate and provide drug usage reports to pharmaceutical partners as requested.
- Comply with Prescription Monitoring Program (PMP) reporting requirements in accordance with state laws.
- Plan and organize pharmacy service activities to meet patient needs and adhere to physician requests, administrative policies, and best practices.
- Participate in continuing education activities to improve pharmacy services and maintain professional development.
- Ensure development, enforcement, and periodic updates of pharmacy policies and procedures.
- Engage in quality assurance initiatives focused on patient care and medication safety.
- Monitor documentation accuracy and assess the effectiveness and safety of drug therapies.
- Assist management in ensuring compliance with all applicable State and Federal pharmacy laws and regulations.
Qualifications:
- Graduate of an accredited school of Pharmacy
- Valid Texas Pharmacist license in good standing (additional state licenses are advantageous)
- USP 797 sterile compounding experience is required
- Proven infusion/IV experience is required
- Intrathecal experience is a plus
