Clinical Pharmacist - Rochester, New York

A direct hire opportunity is available for a Home Infusion Staff Pharmacist based in Rochester, NY. This role offers a Monday through Thursday schedule from 8:00 AM to 5:00 PM, providing an excellent work-life balance for pharmacy professionals experienced in clinical and infusion settings.

Key Responsibilities:

• Review infusion orders and patient therapies to ensure accuracy and safety.
• Participate actively in clinical rounds, contributing expert pharmacy insights.
• Perform sterile compounding and review total parenteral nutrition (TPN) formulations.
• Maintain comprehensive patient records and individualized care plans.
• Ensure strict adherence to USP standards and cleanroom compliance protocols.
• Provide training and ongoing support to pharmacy staff.
• Drive operational improvements to enhance service quality and efficiency.

Qualifications and Desired Experience:

• Current licensure as a Registered Pharmacist in the State of New York.
• Minimum of two years of recent experience in an acute care setting, or at least one year of recent home infusion therapy experience preferred.
• Strong knowledge and practical experience in intravenous admixture services and intravenous therapy.
• Ability to evaluate and monitor drug therapy considering the overall needs and clinical conditions of home care patients, collaborating effectively with physicians, nurses, and other healthcare professionals.
• Familiarity with CareTend system is a plus.
