A specialized opportunity is available for a Home Infusion Staff Pharmacist to contribute clinical expertise within a home infusion setting in Syracuse, NY. This direct hire role offers a consistent Monday through Thursday schedule, from 8:00 AM to 5:00 PM, supporting a strong work-life balance.

Key Responsibilities: Review infusion orders and patient therapies to ensure safety and efficacy Participate in clinical rounds to collaborate on patient care strategies Perform sterile compounding and total parenteral nutrition (TPN) review following best practices Maintain detailed patient records and individualized care plans Ensure compliance with USP standards and cleanroom protocols to uphold product quality Train and provide ongoing support for staff to enhance team performance Lead and advocate for operational improvements within the infusion service

Qualifications and Desired Experience: Current licensure as a Registered Pharmacist in the State of New York is required Minimum of two years recent experience in an acute care environment or at least one year in home infusion therapy preferred Demonstrated expertise and strong knowledge in intravenous admixture services and intravenous therapy Proficient in evaluating and monitoring drug therapy tailored to home care patients needs in collaboration with multidisciplinary healthcare teams

System Experience: Familiarity with the CareTend system is a plus
