Clinical Pharmacist in Torrance, CA

A California-licensed Pharmacist is needed to join a remote Medication Therapy Management (MTM) pilot program supporting Medicaid patients. This part-time contract role offers flexible scheduling with 10-20 hours per week, including day shift hours for patient outreach and counseling. Candidates must reside in California and be currently enrolled as an ORP provider under California Medicaid (Medi-Cal).

Location:

• Remote (Must reside in California)

Schedule:

• Approximately 10-20 hours per week
• Flexible hours, day shift for patient contact
• Guaranteed minimum 8 paid hours/week

Responsibilities:

• Conduct comprehensive MTM consultations and medication reviews
• Provide patient counseling to improve medication adherence and understanding
• Identify medication optimization opportunities based on profile reviews
• Accurately document clinical interventions and patient interactions
• Collaborate with program stakeholders to enhance patient outcomes
• Ensure compliance with state and federal pharmacy regulations

Qualifications and Experience:

• Active, unrestricted California Pharmacist license
• Current California resident
• Prior MTM experience required
• Enrollment as a Medi-Cal ORP (Ordering, Referring, and Prescribing) provider is mandatory
• Excellent communication and patient counseling abilities
• Self-motivated and able to work independently in a remote setting
