Home Infusion Clinical Pharmacist - Waukesha, WI

A clinical pharmacist position is available for a contract-to-hire opportunity, requiring significant recent experience in sterile compounding and home infusion. This role is based in Waukesha, WI, and offers a structured weekday schedule with occasional on-call responsibilities.

Schedule:

• Monday to Friday, 8-hour shifts between 7:30 AM and 6:00 PM
• On call every 4th weekend and one night per week

Responsibilities:

• Verify intravenous (IV) medication orders to ensure accuracy and patient safety
• Prepare and mix IV medications in a sterile clean room environment as needed
• Perform various clinical pharmacy duties to support home infusion services
• Collaborate with healthcare teams to optimize patient therapy plans

Qualifications:

• Active and clear Wisconsin pharmacist license
• Minimum of 2 years recent sterile compounding experience, specifically in IV preparations
• Recent home infusion experience of at least 2 years is preferred
• CPR certification is desirable
• Strong understanding of clinical pharmacy best practices and medication safety standards
